In compliance with the provisions of the Federal Law on Protection of Personal Data Held by Private Parties (the “LFPDP”), CORPO E ALMA SPA, SA DE CV, with address at Pegaso Street number 3324, Colonia la Calma in the municipality of Guadalajara, Jalisco, hereby expresses the following Privacy Notice.

COLLECTION OF PERSONAL DATA
At CORPO E ALMA SPA we obtain personal data from our clients and/or suppliers from the Data Owner, by virtue of the relationship generated between the latter, CORPO E ALMA SPA, health professionals, therapists, suppliers, among others. . The Personal Data that is in the possession of CORPO E ALMA SPA is collected in order to provide the correct provision of services, objects of our society and sensitive ones, those related to the state of health, background and clinical history and information about the way of life. Necessary or convenient to provide quality and security service to our clients.

CONSENT OF THE OWNER
For the purposes of the provisions of the LFPDP and other applicable legislation, the Owner states (i) that this Notice has been made known to him by the Responsible Party and (ii) that he has read, understood and agreed to the terms set out in this Notice, by which grants your consent regarding the processing of your personal and sensitive data.

PURPOSE OF THE PROCESSING OF PERSONAL DATA
The Personal Data in possession of Clínic will be used for the following purposes:
a) Purposes necessary to maintain the relationship with the data owner:
– Provision of services such as: Aesthetic massages, therapeutic massages, facial and body treatments.
– Creation, study, analysis, updating and maintenance of clinical files.
– Billing and collection for services.
-Maintenance of records to monitor services.
b) Different purposes: – Studies, records, statistics and analysis of health information. – Maintenance of records for the provision of services in the future. Statistical and market analysis. – Promotion and marketing of clinical, aesthetic and Spa products and services.

TRANSFER
For the provision of medical services, therapy, rehabilitation, diet and nutrition, creation, study, analysis, updating and maintenance of clinical files, Spas. CORPO E ALMA SPA may transfer, inside and outside the country, the personal data in its possession to subcontracted third parties for the purposes indicated in this Privacy Notice. The third parties to whom said data will be transferred are laboratories, clinics, hospitals, research centers, insurance companies, Spas as well as any company in the same corporate group, or, where appropriate, competent authorities.

ARC PROCEDURE AND REVOCATION OF CONSENT
For the exercise of your ARCO Rights and/or the revocation of your consent for the processing of your personal and/or sensitive data by CORPO E ALMA SPA, we would appreciate submitting a request to exercise ARCO Rights.
To: i) the email contacto@corpoealmaspaholistico.com and/or ii) in writing to Calle Pegaso number 3324, Colonia la Calma in the municipality of Guadalajara, Jalisco, from Monday to Saturday from 10:00am - 8:00pm. Both addressed to CORPO E ALMA SPA Calle Pegaso number 3324, Colonia la Calma in the municipality of Guadalajara, Jalisco, accompanied by the following information, within a period of no more than 5 (five) calendar days, counted from the date of obtaining the information. :

1. Name, complete address (street, interior and/or exterior number, neighborhood, zip code, city and state);
2. Identification that proves your personality (INE credential, current passport, professional license and/or immigration document); and
3. If the owner is not the one who presents the request, the document that proves the existence of the representation, that is, a public instrument or power of attorney signed before two witnesses, along with identification of the owner and the representative (INE credential, valid passport, professional ID or immigration document).
4. Reason why you wish to exercise your ARCO Rights and/or the revocation of your consent.
5 Description of personal data on which it is intended to exercise any ARCO right.
6. Date of care and name of the therapist or esthetician from which you received the services.
7. Any other element that allows the location of personal data and attention to the request.

REQUEST FOR ACCESS, RECTIFICATION, CANCELLATION OR OPPOSITION OF PERSONAL DATA AND REVOCATION OF CONSENT (ARCO REQUEST)
All personal data are processed in accordance with applicable and current legislation, therefore the owners of personal data have at all times the right to access the personal data held by CORPO E ALMA SPA and the details of their processing. , as well as rectifying them if they are inaccurate or incomplete; cancel them when they turn out to be excessive or unnecessary for the purposes for which they were obtained; and oppose your treatment.
